You can ask a variety of question types and view reports online or download data to a spreadsheet for analysing.<\/p>\n<h3>Resources<\/h3>\n<ul>\n<li><a title=\"Questionnaires Module\" href=\"https:\/\/docs.moodle.org\/401\/en\/Questionnaire_module\">Questionnaires Module<\/a> (Moodle Docs website)<\/li>\n<\/ul>\n<h2>How to create a Questionnaire<\/h2>\n<ol>\n<li>Log into Moodle.<\/li>\n<li>Navigate to the course and section where you&#8217;ll be adding the Questionnaire.<\/li>\n<li>Make sure editing is turned on (<em>(<\/em><em>Turn editing<\/em>\u00a0<em>on<\/em> \u2013 toggle icon in the top right hand corner)) and click the\u00a0<em>Add an activity or resource<\/em> link in the bottom right hand corner of the section.<\/li>\n<li>Select\u00a0<em>Questionnaire<\/em> from the list and click <i>Add<\/i>.<\/li>\n<li>Give the\u00a0<em>Questionnaire\u00a0<\/em>a name and any other settings that apply:\n<ol>\n<li><strong>Availability <\/strong>&#8211; you can use these settings to limit access to the questionnaire. The questionnaire\u00a0will always be visible but if outside of the open and close dates students will not be able to make contributions and will be told the questionnaire\u00a0is closed.<\/li>\n<li><b style=\"color: #000000\">Response options\u00a0<\/b><span style=\"color: #000000\">&#8211; you can use these settings to make the\u00a0questionnaire anonymous or allow students to make multiple responses. You can also allow branching questions here, meaning questions would be tailored to earlier responses. Very important to look at the setting for\u00a0<\/span><em style=\"color: #000000\">Students can view ALL\u00a0<\/em><i>responses<\/i><span style=\"color: #000000\"> &#8211; if you don&#8217;t want students to view all of the responses to a questionnaire make sure this is set to\u00a0<em>After the questionnaire is closed<\/em> and that you don&#8217;t have a\u00a0<em>Close date<\/em> set in the\u00a0<em>Timing<\/em> section (from above).<\/span><\/li>\n<li><strong>Content options<\/strong> &#8211; you can choose to copy an exisiting\u00a0questionnaire with these options.<\/li>\n<li><strong>Common module settings<\/strong> &#8211; use these settings to show or hide the questionnaire, as well as deciding if you need to view the results by groups (use Separate Groups if you want to see how each group of students responded). Use groupings if you want to limit the results to a certain set of groups &#8211; say only EPE groups.<\/li>\n<li><strong>Restrict access<\/strong> &#8211; use these settings to restrict access to the questionnaire based on a rule or set of rules. These rules can be based on activity completion, date, grades, or a setting in the user profile. This is useful if you want the\u00a0questionnaire to only be shown after a certain time.<\/li>\n<\/ol>\n<\/li>\n<li>Click\u00a0<em>Save.<\/em><\/li>\n<\/ol>\n<h2>How to\u00a0add questions to a Questionnaire<\/h2>\n<ol>\n<li>Log into Moodle.<\/li>\n<li>Navigate to the questionnaire within your course and click on it.<\/li>\n<li>From the\u00a0<em>Questionnaire administration<\/em> panel (usually a grey panel to the right hand side of the page) select\u00a0<em>Questions.<\/em><\/li>\n<li>Select a question type from the drop down menu and click\u00a0<em>Add selected question type.<\/em>\n<ol>\n<li><strong>Page Break<\/strong> &#8211; creates a page break, questions following this will be presented to students on a new page.<\/li>\n<li><strong>Check Boxes<\/strong> &#8211; use this\u00a0for multi choice questions (can select more than one answer).<\/li>\n<li><strong>Date\u00a0<\/strong>&#8211; use this for answers in date format (dd\/mm\/yyyy).<\/li>\n<li><strong>Dropdown Box<\/strong> &#8211; use for answers with a dropdown menu (only one answer is possible).<\/li>\n<li><strong>Essay Box<\/strong> &#8211; use for long text answers.<\/li>\n<li><strong>Label<\/strong> &#8211; creates a label area in the questionnaire with no associated question.
